O Scale refers to the scale ratio of model trains, typically 1:48 in the United States, representing the proportion to the actual train. O Gauge, on the other hand, refers to the distance between the outer rails of the track, which is 1.25 inches (31.75 mm) for O Gauge. While O Scale pertains to the size of the models, O Gauge pertains to the track width. They are often confused by people who don't know how to distinguish those yet.....so that's why even on this site we typically will put O Scale / O Gauge in the titles to cover how people will search/identify with the products and what they are looking for.
